Abstract

Uspostava helikopterske hitne medicinske službe (engl. Helicopter Emergency Medical Service - HEMS), za razliku od službe traganja i spašavanja (engl. Search And Rescue - SAR) nije obveza države, ali na današnjem stupnju civilizacijskog razvoja zemalja zapadnog svijeta njeno postojanje se podrazumijeva i ne smatra se nad standardom zdravstvene usluge. Kako se Republika Hrvatska smatra dijelom zemalja razvijenog zapadnog svijeta i kao takva teži osigurati što bolje uvijete zdravstvene skrbi... More svojim građanima na cjelokupnom svom teritoriju, uspostava helikopterske hitne medicinske službe od vitalnog je značaja za unapređenje svih aspekata života u Republici Hrvatskoj. Postojeći sustav hitne medicinske pomoći je već odavno potkapacitiran i preopterećen te kao takav, što zbog geografskih specifičnosti Republike Hrvatske, što zbog sve manjeg broja raspoloživih liječnika na tržištu rada, nedostatan i neadekvatan za pružanje brze i učinkovite hitne pomoći u okvirima „zlatnog sata”. Kako se u zadnjih petnaestak godina cestovna infrastruktura značajno unaprijedila i enormno povećala količinu prometa u slabo naseljenim područjima gdje je udaljenost između dvije najbliže bolnice nerijetko veća od 100 km od iznimnog je značaja potreba uspostave helikopterske hitne medicinske službe kako zbog smanjenja posljedica prometnih nesreća tako i zbog poboljšanje medicinske skrbi u posebnim situacijama – kardiovaskularnim problemima, medicinskim i spasilačkim intervencijama na izoliranim mjestima te ostalim kritičnim situacijama, prema zahtjevima „zlatnog sata”. Svako daljnje odlaganje osiguravanja takve usluge značajno bi intenziviralo iseljavanje ruralnih prostora i ograničilo perspektive turističkog razvoja. U ovom radu su analizirane prednosti i ograničenja helikopterske hitne medicinske službe kao i potrebni administrativni i infrastrukturni zahvati kao preduvjet za uspostavu iste u Republici Hrvatskoj, te je elaboriran optimalan prijedlog organizacije takvog sustava. Abstract (english)

Establishing a Helicopter Emergency Medical Service (HEMS), unlike the Search and Rescue Service (SAR), is not an obligation of the state considered above the standard of health service. As the Republic of Croatia is considered to be part of the developed Western world and as such seeks to ensure the best possible health care conditions for its citizens throughout its territory, the establishment of a helicopter emergency medical service is vital for the advancement of all aspects of life... The existing emergency medical care system has long been undermined and overburdened, and as such, which, due to the geographical specificities of the Republic of Croatia, which, due to the decreasing number of available doctors on the labour market, is insufficient and inadequate to provide quick and effective ambulance within the “golden hour”. As road infrastructure has significantly improved over the last fifteen years and enormously increased the amount of traffic in sparsely populated areas where the distance between the two closest hospitals is often greater than 100 km, the need to establish a helicopter emergency medical service is of utmost importance, both due to the reduction of the consequences of traffic accidents and improving medical care in special situations - cardiovascular problems, medical and rescue interventions in isolated locations, and other critical situations, as required by the "golden hour". Any further delay in providing such a service would significantly intensify rural displacement and limit the prospects for tourism development, and an optimal proposal for the organization of such a system has been elaborated.
